HOW DO I MAKE AN APPOINTMENT WITH DR. DONA?
You can use the "Contact Us" form or submitting the new patient form on the homepage. You can also either send an email to Dr. Dona, who will respond to you within 24 hours. You can also reach us directly at the office at (813-515-9629.) We will return your call promptly. This usually includes a 15 minute phone call to see if you or your family member would be a good fit for our clinic.
HOW DO I PREPARE FOR THE FIRST SESSION WITH DR. DONA?
Once an appointment has been made, you will be emailed access to our patient portal From there, you will fill out a new patient questionnaire, sign clinic policies, consents for treatment and release of information. The questionnaire includes general information about your/or your child’s background, psychiatric and medical history.
We understand that these forms can seem lengthy and take time to complete, but completing these prior to the initial interview will help Dr. Dona prioritize any areas of concern and help with overall assessment. We request you complete these questionnaires 24 hours prior to your initial evaluation appointment.
WHAT IF I NEED TO CANCEL OR RESCHEDULE AN APPOINTMENT?
Please cancel or reschedule at least 48 business hours prior to the appointment. Your appointment time is reserved only for you and / or your child. Making appointments last minute make it difficult to schedule another patient in need at that time. Letting us know with enough advanced notice allows us to make that appointment time available to another patient in need. If you cancel or no show within 48 hours of the appointment, you will be charged the fee for the whole session.
If you fail to cancel or reschedule a full session fee will be billed for the time.
HOW CAN I PAY FOR APPOINTMENTS?
Dr. Dona accepts cash, venmo, zelle, and all major credit cards for all professional services.
All outpatient visits must be paid in full at the time of visit, unless other arrangements are made prior to the appointment.
HOW CAN I USE MY MEDICAL INSURANCE TO PAY FOR SERVICES IF DR. DONA DOES NOT TAKE INSURANCE?
Dr. Dona is not a part of any insurance panels and cannot process directly for insurance reimbursement. She is considered an “out of network” or “non-participating” provider for most insurance plans.
If you happen to have health benefits that provides mental health coverage with “Out of Network” benefits, you can be eligible for insurance reimbursement for any provided professional services.
Several patients submit their invoices from sessions with Dr. Dona directly to their insurance companies to receive reimbursement according to the terms of their insurance.
Patients are advised to inquire with their insurance about coverage for out of network providers for mental health to see what benefits may be available to them.
Dr. Dona can provide you a superbill which you can submit to your insurance. Please check with your medical insurance regarding the terms of your policy and verify out-of-network benefits and reimbursement rates.
Potential reimbursement is between you and your insurance carrier, and you may discuss this with your insurance company by contacting them directly. Regardless of the patient’s ability to receive insurance reimbursement, full payment for all services is required at the time of each appointment, unless other arrangements are made.
DO YOU OFFER GENESIGHT OR OTHER TESTING?
Yes we do offer Genesight testing which depending on your insurance may cost an additional fee. We also offer standardized ADHD testing.
